Step 1. Request in
writing a NV Medical Marijuana application package along with a $50
money order or cashier’s check to: NV Dept. of Health, MMP Registry,
4150 Technology Way Ste. 104, Carson City, NV 89706. In order to
qualify for the program a patient must have a medical condition with
the symptoms of chronic pain, nausea, muscle spasms or seizures.
This automatically qualifies cancer, glaucoma, migraines, degenerative
disk disease, back problems or any other condition with these symptoms.
Medical records recommended, but not always necessary, as the doctor
will diagnose conditions at the time of consultation.
Step 2. Doctor’s Appointment. Our staff will meet
you at the Dr.’s office and assist you with the medical marijuana
program, providing you with all the information you need to know pertaining
to the program. If you have any questions about what paperwork you
need, or if you’re not sure your condition qualifies, please call
702-252-3500 and ask our staff. You will also have your recommendation
signed by the doctor and notarized by us ready to be submitted to
the Health Dept. in Carson City. Doctor's fee for signing your application
$200.
We notarize your Waiver and Acknowledgement form, fee $10.
These above fees are to be paid upon the appointment.
Step 3. Once the application is completed it must be sent to the Nevada State Health Division, Medical Marijuana Program, 4150 Technology Way Suite 104, Carson City, NV 89706. At this time they will charge a background check fee. The background check takes approximately 30-45 days. Make money order/cashier’s check to: Health Department Background fee $150.
Step 4. Get your finger prints taken, fee $20.
Step 5. Final registry card at DMV $11-$22.
